HPV biology divides PSCC into clinically important molecular groups. The overall HPV prevalence reported across 52 studies is 50.8%, while p16 immunohistochemistry positivity has been reported in 41.6% of penile cancers.

These biomarker data support additional research into which patients derive the greatest benefit from checkpoint inhibition.
PRIAM
The PRIAM Phase II trial is evaluating carboplatin/paclitaxel plus pembrolizumab as induction treatment for node-positive locally advanced penile squamous cell carcinoma.
The study plans to recruit 50 patients and uses pathological complete response as a primary efficacy endpoint. It will also evaluate PD-L1, high-risk HPV, progression-free survival and overall survival.
Sintilimab Plus Chemotherapy
A prospective 2026 study of perioperative sintilimab plus paclitaxel and cisplatin reported a 52.9% objective response rate in evaluable neoadjuvant patients.
The results support further investigation of PD-1 inhibition in resectable but locally advanced penile cancer.
Toripalimab and EGFR Combination Strategy
EAU-ASCO cites a Phase II Chinese study evaluating toripalimab, the EGFR-directed antibody nimotuzumab and triplet chemotherapy before surgery.
Among 24 patients who underwent consolidative surgery, 14 achieved a pathological complete response. Two-year overall survival was 72.4% and progression-free survival was 65.5%.
The multi-agent design makes it difficult to determine the contribution of each component, but the response rates justify continued investigation.
EGFR-Directed Treatment

EAU-ASCO reports that dacomitinib produced an objective response rate of 32.1% in 28 patients, including one complete response.

Penile Cancer Treatment Market Competitive Landscape
Penile cancer remains one of the least commercially consolidated oncology treatment markets. Competition is distributed across platinum chemotherapy, immune checkpoint inhibition, EGFR-targeted treatment, tumor-agnostic precision oncology, surgery and radiotherapy.
Chemotherapy remains the established systemic leader, but immune-oncology is becoming the principal pharmaceutical innovation area. Merck has gained clinical visibility through HERCULES and PRIAM, while Bristol Myers Squibb, Roche, Incyte, Innovent and Shanghai Junshi have generated additional checkpoint-inhibitor data.
The next competitive transition is likely to depend on perioperative treatment. A therapy capable of converting bulky node-positive disease into resectable disease, increasing pathological complete response and improving long-term survival would occupy a high-value treatment position.
Molecular stratification will create smaller commercial niches around MSI-H/dMMR, TMB-high, HER2-positive and other actionable tumors. Because these biomarker-defined groups are uncommon, global trial networks and centralized diagnostic testing will remain essential.
